Diakon Lutheran Social Ministries is one of the premier employers in Pennsylvania and Maryland. We are a non-profit organization which includes Diakon Senior Living Services and Diakon Child, Family, and Community Ministries.

 

General Responsibilities:

  •  Assists families in obtaining supportive services from the community to help them remain together as well as provides daily activities and special events as follows:
    •  Refers and links the families to supportive services provided by the Community such as, mental health services, drug and alcohol treatment facilities, housing resources, utility assistance, transportation, counseling, food banks and legal advocacy.
    • Educates families on service availability, application procedures, client rights, etc.
    • Makes regular contact with families as well as facilitates family meetings to help problem solve.
    • Provides case management when service providers are not available through the general community.
    •  Maintains availability to families on a rotating 24-hour basis for emergency and odd hour coverage.
    • This position will be serving the Bristol Borough School District., Bucks County.
    • Childline and DHS FBI clearance results are required prior to beginning employment.
    • Mandatory Covid-19 Vaccination Program-need covid vaccine card or file for an exemption.

Education/Training: A BA or BS degree in social work, BSW, normally required. 

 

Experience: A minimum of two (2) years’ experience in a related child welfare or human service position normally required.  Experience in social work or case management is essential.

 

ADA: Ability to stand, bend, kneel, stoop, climb, push, and pull items weighing up to 50 lbs.; Valid driver’s license; Visual and auditory skills.
